Correct a signed note

Fix a mistake in a signed note, or in the prescription made from it, by signing a correction. The original stays in the record.

KeelDoctorOwnerguideChecked 2026-10-11

Purpose

A signed note can't be edited. To fix a mistake, such as a wrong dose, you sign a correction. Keel keeps the version you replace and shows what changed, so the record still explains itself.

Before you start

the same people who can sign the note can correct it: the doctor who ran the consultation, an Owner with prescribing rights, or a doctor covering for them. Backup signers give a reason.
a correction is a new signature. Keel checks the medicines again before it's signed.

Steps

Step 1: Open the signed note

Doon the patient's appointment page, select View note in the Consultation card.
You'll seethe note with SIGNED at the bottom and Amend in the top bar.

Step 2: Select Amend

Doselect Amend.
You'll see"This correction replaces the signed note. The original is kept in the record." and the keys Discard correction and Sign correction.
Soyou can now edit the note. Nothing changes in the record until you sign.

Step 3: Make the correction

Dochange only what's wrong, for example the dose in the medicines table.
to stop without changing anything, select Discard correction. The signed note stays as it was.

Step 4: Sign the correction

Doselect Sign correction. If you're signing for another doctor, give a reason in the dialog and select Sign correction there.
You'll seeSigning…, then the note is read-only again.
a backup signer sees "Why are you correcting this note?" or "Why are you correcting" the doctor's note, with the same quick reasons as signing.

Step 5: Check the amended note

Dolook at the line at the bottom of the note, and at each box you changed.
You'll seeAMENDED → by your name and the time, then replaces the version signed by the earlier signer and time. Each changed box shows Amended and Was: with the old text.

Result

You'll seethe corrected note as the current record, with the replaced version's text shown under each changed box.
Soanyone reading the note can see what was corrected, by whom and when.

If it doesn't work

  • If you see: Amend is greyed outHover over it to see why. The fixes are the same as for signing (Why signing is blocked).
  • If you see: Not signed · a medicine needs you, or a medicine marked with a blue barFix the medicine, then sign the correction again (Resolve medicine review blockers)
  • If you see: "Add a reason before signing the correction."Select Add a reason and fill it in
  • If you see: "Another change landed first. Sign the correction again."Select Try again
  • If you see: "Could not reach the server. Nothing was saved — the signed note is unchanged."Check your connection, then select Try again
  • If you see: "This note was signed somewhere else. Reload to see the current version — your correction will not be kept."Select Reload, check the latest version, and correct it again if you still need to
  • If you see: "Your permission to correct this note changed. Reload to see the current version — your correction will not be kept."Select Reload. Your cover or prescribing rights may have ended.
  • If you see: "The patient withdrew consent for this recording, and the note was deleted. There is nothing to correct."Select Close
  • If you see: "Something went wrong. Nothing was saved — the signed note is unchanged."Try again later. The signed note is still the record.
  • Working: AMENDED at the bottom of the noteDone

Limits

a correction can't add or remove boxes. It changes what's in them.
a prescription can't be edited on its own. To fix one, correct its signed note.
the corrected prescription carries the name, qualifications and registration of whoever signed the correction, and a new QR code.
opening or printing the corrected prescription from Keel is not available yet. Open prescription appears only right after a note's first signature.
Keel doesn't recall or mark a prescription you already handed over.
correcting isn't available in the Keel phone app. A signed note there says "To correct this or print the prescription, open it on the web."
